All forms may be emailed, mailed, or faxed. Directions listed below.
(1). Diver's Insurance
All Indiana Diving Academy campers must have current diver's insurance. If you’re already a USA DIVING member, please provide us with a copy of your membership card or membership confirmation receipt. To obtain the required insurance, go to www.usadiving.org and "Join USA Diving." For participation in camp, a “Limited Athlete Membership” is all you need. Once you have your membership information, please include a copy of your USA DIVING membership card or membership confirmation receipt with the rest of the required forms. Please Note: We DO NOT accept AAU memberships.
(2). Medical Consent Form:
No camper will be allowed to check-in for camp without fully completing this form. The Medical Consent Form ensures that your son/daughter receives the best, most prompt care in the event of an illness, injury, or emergency. While we hope that your son/daughter has no issues while at camp, Indiana University and the City of Bloomington have excellent health care facilities.
(3). Release of Liability Agreement:
In order to participate at the Indiana Diving Academy, a Parental Consent and Release of Liability Agreement must be signed and on file. Please download the agreement and return to the Indiana Diving Academy.
(4). Diving Safety Form:
Attention Divers and Parents. U.S. Diving (Gabriel, 1990) recommends that all U.S. Diving coaches provide divers and parents with a warning of the potential dangers inherent within the sport of diving. Please download, sign, and return the Diving Safety Form to the Indiana Diving Academy.
(5). Current Diving List & New Dives To Learn:
For you to get the most out of the Indiana Diving Academy, we would like to know more about your current diving list, experience, competition level, and dives you would like to learn prior to your coming to camp. Please tell us all the dives you can currently complete for each level. Complete the Current Diving List & New Dives To Learn form and send it in. If you run out of room, please write them on the back of the page.
